Managing Uncontrolled Symptoms

Should you develop uncontrolled symptoms that cannot be addressed in your current residence, your hospice benefit includes General Inpatient Level of Care provided in a hospice inpatient unit or skilled nursing facility. We may also provide you with a medication kit of prescribed medications to be kept in your home in the event of a medical crisis.

In order to continue to receive care from us, any hospitalization related to your diagnosis must be provided at an MJHS Hospice partner facility. If your symptoms become unmanageable, call one of our 24/7 numbers.

Medication Care Kit

The kit includes medication to have on hand to help you feel comfortable if you are experiencing pain or discomfort and will eliminate the wait time associated with going to or calling a pharmacy for delivery. The medications in your kit will depend on your symptoms and diagnosis.

The kit will be delivered by our pharmacy provider or to your home within a day of starting hospice care. It should be placed in the refrigerator immediately after it arrives. The medications should be used only under the direction of the RN Case Manager or prescriber. These medications are for the patient only.


Be Prepared

Speak to your RN Case Manager at the time of admission to predetermine where you would prefer to be transferred, should you need and additional level of care. It is better to be prepared and inform family members to avoid added stress.
